Plot weight, length or height and relevant head circumference on an appropriate chart. Use reliable technique and consider corrected age after prematurity according to the chart guidance. A measurement error can create an apparent growth problem, so check unexpected results before drawing conclusions.
Interpret serial growth alongside birth size, parental stature, nutrition and health. Crossing centiles may require assessment but is not a diagnosis by itself. Review feeding, chronic symptoms and social circumstances; a stable small child and a child with falling growth need different reasoning.
Assess gross motor, fine motor, language, social and adaptive abilities through history and observation. Milestones have a range, and hearing or vision difficulties can affect performance. Consider opportunities and language environment without dismissing persistent concerns as merely individual variation.
Loss of previously acquired skills needs prompt assessment, particularly with seizures, weakness or abnormal neurological findings. Distinguish regression from slow acquisition or a temporary effect of illness. Do not advise prolonged watchful waiting when a child is clearly losing function.
Refer through developmental, hearing, vision and therapy pathways as indicated. Intervention need not wait for every diagnostic label. Agree meaningful functional goals with families, coordinate school or nursery support and arrange follow-up so concerns and referrals do not disappear between services.
Read the relevant guideline sections alongside these original clinical teaching cases. Local pathways, available services and individual patient factors determine care.
