Fetal Growth, Movements & Surveillance

Recognise growth concerns and interpret fetal wellbeing assessments in clinical context.

Quick revision ↓   Medical Dictionary

01Growth assessment

Assess gestational dating before interpreting size. Serial clinical assessment and ultrasound are used according to risk and findings. A small fetus may be constitutionally small or growth restricted; a centile alone does not establish the cause. Review the growth trajectory, maternal conditions and placental function rather than relying on a single estimated weight.

02Changes in movement

Teach awareness of the usual movement pattern and prompt contact with maternity services if movements reduce or change. Do not advise waiting for the next routine visit when concern is present. A home Doppler or a brief return of movement does not replace appropriate assessment. The evaluation depends on gestation and the clinical picture.

03Ultrasound and Doppler

Ultrasound estimates growth and fluid volume, with Doppler studies assessing selected circulatory patterns when indicated. Measurements have uncertainty and should be interpreted with the interval between examinations. Abnormal results may alter surveillance or delivery planning. Explain the purpose and limitations of tests, avoiding a claim that a normal scan guarantees every aspect of fetal health.

04Surveillance decisions

Cardiotocography and other surveillance tools answer particular questions within a broader assessment. Consider symptoms, maternal observations, gestation, growth and previous results together. A concerning trace or worsening maternal condition requires timely senior review. Repeated testing should have a stated purpose and a plan for action if findings deteriorate.

05Multiple pregnancy

Establish chorionicity and amnionicity early where possible because shared placentation affects risk and monitoring. Twins do not all follow the same surveillance schedule. Specialist pathways address growth discordance and complications of shared placentation. Birth timing and route are individual decisions that consider fetal presentation, gestation and the condition of both mother and babies.

High-Yield Revision

  • A growth centile is a finding, not a complete diagnosis.
  • Changed movements deserve prompt maternity assessment.
  • Chorionicity determines important twin-pregnancy risks.

Common Pitfalls

  • Using a home Doppler to dismiss reduced movements.
  • Treating every twin pregnancy as having identical risk.
