Preconception & Routine Antenatal Care

Plan prevention, screening and individualised monitoring from before conception through pregnancy.

Quick revision ↓   Medical Dictionary

01Prepare before pregnancy

Review chronic disease, medicines, vaccinations, nutrition and previous pregnancy outcomes. Discuss folic acid using the appropriate risk-based local recommendation and avoid unreviewed potentially harmful medicines. Do not stop essential treatment abruptly; coordinate safer alternatives with the responsible team. Preconception care also addresses smoking, alcohol, mental health and practical access to services.

02Booking assessment

Early antenatal contact allows dating, risk assessment and planning. Record medical, obstetric, family and social history with baseline observations and relevant screening. Blood group, antibody status and infection screening have specific follow-up implications. Explain what each test can and cannot show, and support informed acceptance or decline without treating screening as compulsory.

03Dating and screening

Ultrasound helps establish gestational age and assess pregnancy features, while screening estimates risk rather than confirming every condition. A positive screening result may require counselling and diagnostic options. Accurate dates influence later decisions about growth, preterm birth and post-term care. Record uncertainty when menstrual dates and imaging do not initially agree.

04Continuing review

Antenatal care includes blood pressure, symptom review and appropriate fetal growth surveillance. The schedule varies with risk and local practice. Ask about emotional wellbeing and barriers to attendance. Provide clear advice about bleeding, fluid loss, severe headache, fever, significant pain and reduced fetal movement rather than relying on scheduled appointments alone.

05Shared planning

Discuss place of birth, support, analgesia and anticipated care as pregnancy progresses. Revisit plans if risk changes. Explain relevant immunisation and nutrition advice from current local guidance. Every referral, abnormal screening result or requested repeat test needs ownership. Good antenatal care combines preventive checks with a relationship in which concerns can be raised promptly.

High-Yield Revision

  • Early booking supports risk assessment and planning.
  • Screening estimates risk; diagnostic testing answers a different question.
  • Medication changes should be coordinated.

Common Pitfalls

  • Stopping essential medicines abruptly.
  • Treating scheduled visits as a substitute for urgent symptom review.
